When you schedule an annual Medicare Wellness appointment at CRMC, you’ll receive more than just a conversation about physical health, nutrition, and exercise. The Medicare Wellness Program takes a multidimensional approach to health and wellness including checking on your emotional, social, and intellectual wellness.

What does an annual wellness visit include?

A visit includes routine measurements, review of individual and family history, medication and supplement review, discussion of chronic condition care, review functional ability and safety, referrals to education or counseling, and planning a schedule for the screening and preventative services needed the next five to 10 years.

What is preventative care and why do I need it?

Preventative care is what we do to prevent or delay medical problems. Getting preventative care saves you the time, money and the worry that comes with medical problems. And, if you avoid medical problems, you’ll be more independent and have a better quality of life for a longer time.


Do I have to pay for this visit?

No, Medicare pays the entire cost of the Annual Wellness visit (AWV) -once every 365 days. Please be aware that during the course of your AWV, your provider may discover and need to investigate or treat a new or existing problem. 

This additional care is considered diagnostic, meaning your provider is treating you because of certain symptoms or risk factors. Yearly Medication Review also falls under this category. 

Medicare may bill you for any diagnostic care you receive during a preventive visit. If you have Supplemental Insurance, it may cover many of these diagnostic services.


Why do I need a personalized plan?

Preventative care is not the same for everyone. What’s best for you depends on how you live and upon you and your family’s medical history. 


What will happen during the visit?

During your Annual Wellness Visit, we’ll gather more information about your unique health situation. We’ll discuss what medical problems you may need to watch out for and how we can prevent them. You may be given some preventative care, such as a flu immunization, but you will not be given an actual physical exam


What if my health situation changes in the future will my prevention plan change too?

Yes. Medicare will pay for you to come in every year and update your plan according to your current needs.


If I have other health concerns, can I ask about them during the visit?

If you have specific concerns about your health, please bring them to our attention, but you need to schedule another appointment with your provider.


What should I bring to my planning visit?

When you come in for your Medicare Annual Wellness Visit, please bring the following:

  • Any medical records you have, including immunization records.
  • Any family health history, in as much detail as possible.
  • A complete list of medications you take, including vitamins, supplements and herbs. For each medication, write down how often and how much of each you take.
  • A list of healthcare providers and suppliers currently involved in your care.
