Medical Records
The Health Information Management department manages medical records and is responsible for timely processing, retrieval, coding, transcribing, and storage of medical records of patients receiving services at CRMC. If you have an urgent medical record need, please call 218-545-4466 so arrangements can be made for quick processing of your request.
Accessing Your Medical Records
You can access your medical records through the MyChart patient portal. If you don’t already have access to MyChart, you can register here.
If you need to release your medical records for worker’s comp, disability determination, an insurance claim, or any other reason, follow these steps:
- Download the Authorization for Disclosure of Health Information form. Or call 218-545-4466 to request they mail you a hard copy.
- Complete the form and email it to roi@cuyunamed.org or fax to 218-546-6091. You can also mail the form to CRMC Health Information Management, 320 East Main St., Crosby, MN 56441.
