DOT Physicals at Baxter and Longville Clinics
There’s no need to detour, we’re offering Certified DOT (Department of Transportation) Physicals at our Baxter and Longville Clinics.
A DOT physical exam is required for commercial drivers to ensure they’re healthy enough to operate safely. It’s a detailed screening of their physical and mental fitness for the job. The exam includes a review of the driver’s medical history and medications, along with tests to confirm they meet the health standards for driving long distances.
What to Bring to a DOT Physical
To help your DOT physical go smoothly, consider filling out the driver’s portion of the exam form at home. On the day of your appointment, make sure to bring the following:
- A list of all your current medications, including dosages
- A list of all your healthcare providers, including names and addresses
- Glasses, contact lenses, or hearing aids, if you use them
- Recent blood sugar logs and your latest HbA1C results (if you have diabetes)
- CPAP machine compliance report showing at least 90 days of use (if you have sleep apnea)
- A clearance letter from your cardiologist confirming you’re safe to drive (if you have a heart condition)
- Results from any recent heart tests (e.g., stress test, echocardiogram)
- A letter from your neurologist if you’ve had a stroke, seizure, brain tumor, or brain bleed
- A letter and medical records from your doctor if you’re taking medications that cause drowsiness or are controlled substances
- A letter from your doctor outlining any permanent limb loss and any related work restrictions
- If you take warfarin (Coumadin), a clearance letter and your most recent INR (blood level) test results
Bringing complete documentation can help avoid delays and ensure your exam is processed quickly.
