Planning ahead helps reduce stress later. You can:

  • Log into MyChart to estimate your out-of-pocket costs based on your insurance.
  • View our Price List Spreadsheet for standard rates by service.

Uninsured? Request a Good Faith Estimate

If you are uninsured or paying out of pocket, you have a right to request a written estimate of your medical charges before receiving care. This is called a Good Faith Estimate. You can request this estimate before scheduling care. If your bill is $400 or more above the estimate, you can dispute it. This estimate must be provided at least one business day before your service and include all related costs like tests, prescriptions, and fees. Keep a copy of your estimate for reference. Know Your Insurance Benefits

Every insurance plan is different. Check with your insurance company to understand how these items apply to you:

Preventive vs. Diagnostic Visits
A routine physical may be covered 100%, but if your doctor orders tests, you might have to pay more.

Referrals and Prior Authorizations
Some services need a referral or approval from your insurance before you get care.

In-Network Coverage
“In-network” means your insurance has an agreement with certain doctors and hospitals for lower rates. Call your insurance company to make sure CRMC is in-network for your plan.

Be Prepared to Pay for Copays or Deductibles

At the time of your visit, you may be asked to pay a copay (a set fee for a specific service) or part of your deductible (the amount you pay out-of-pocket before your insurance starts covering costs).

Register for MyChart

Registering for MyChart ahead of your appointment helps you save time and stay informed. With MyChart, you can:

  • View your upcoming appointment details
  • Fill out forms and check in online
  • Send secure messages to your care team
  • Access test results, billing info, and more

If You’re Responsible for a Dependent Age 12 or Older

Starting at age 12, minors have certain privacy rights related to their healthcare. This means additional paperwork may be needed, including the minor’s Social Security number and signature to allow parent access to medical records or MyChart. Some information may be private, even from parents.

Out-of-Network or Emergency Care Protections

If you receive emergency care or are unexpectedly treated by an out-of-network provider at an in-network facility, you’re protected from surprise medical bills. This means you can’t be charged more than your in-network cost-sharing amount (like a copay or deductible) for most emergency services or certain hospital-based services (like anesthesia or radiology).

By law, out-of-network providers can’t bill you for the difference between what your insurance pays and what they charge, unless you sign a written agreement to waive your protections (which you’re never required to do).

Your insurance plan must:

  • Cover emergency services without prior approval
  • Pay out-of-network providers directly
  • Apply what you pay toward your deductible and out-of-pocket maximum
