Health Care Directive Options
CRMC offers both short and long health care directive forms, each legally valid in Minnesota. The main difference is the level of detail. Long Form
- A more detailed document.
- Includes space for specific treatment preferences such as life-sustaining measures, pain management, and organ donation.
- Allows for more personal statements and nuanced instructions.
- Best if you want to clearly outline your values, goals, and specific wishes.
Short Form
- A simplified version.
- Focuses on naming a health care agent (someone to make decisions for you).
- May include brief instructions about your care preferences.
- Ideal if you want a quick, straightforward directive.
