Health Care Directive Options

CRMC offers both short and long health care directive forms, each legally valid in Minnesota. The main difference is the level of detail. Long Form

  • A more detailed document.
  • Includes space for specific treatment preferences such as life-sustaining measures, pain management, and organ donation.
  • Allows for more personal statements and nuanced instructions.
  • Best if you want to clearly outline your values, goals, and specific wishes.
Long Form Healthcare Directive

Short Form

  • A simplified version.
  • Focuses on naming a health care agent (someone to make decisions for you).
  • May include brief instructions about your care preferences.
  • Ideal if you want a quick, straightforward directive.
