On-site preventative care

Cuyuna Regional Medical Center’s Geriatric Physician Services is a team of geriatrics specialists and a nurse practitioner who provide primary care medical services to those in the Care Center. We strive to meet the unique and growing needs of complex, frail, and special needs patients. Our care model is designed to help patients and their families avoid stress by eliminating time and travel to clinic appointments. On-site preventative care provided on an ongoing basis has been shown to reduce the need for emergency care and trips to the hospital. Our on-site preventative care also helps to significantly improve overall health and quality of life through collaboration with medical professionals who work on the team including nurses, social workers, nutritionists, physical therapists, occupational therapists, pharmacists, and psychiatrists.
