Community care is provided by visiting nurses and other health care providers who see patients in a personal residence or care facility. Our professionals provide care from prenatal and birth, to chronic disease management, to end of life. A Community Paramedic travels to people’s homes to provide follow up care that helps them live a healthier life. Home Health nurses visit adults with acute and chronic medical conditions and Hospice nurses provide exceptional end-of-life care focused on easing the physical, emotional, and spiritual pain that can accompany terminal illness. CRMC provides remarkable clinical expertise and truly comforting patient care.

Community Paramedic

Rather than racing to the scene of an emergency and acting quickly to save lives, CRMC Certified Community Paramedics and Respiratory Therapists methodically travel to people’s homes when there is not a crisis to help them live a healthier life.

Home Health & Hospice

We provide patients (and their families) with a uniquely customized team of doctors, nurses, home health aides, therapists (physical, occupational and speech), social workers and homemakers.
