Questions About Your Medications?
Patients and caregivers meet one-on-one with a clinical pharmacist for an in-depth review of all medications including: prescription, over the counter, and herbal/nutritional supplements. The pharmacist will assess all medications for indication, effectiveness, safety, and convenience.
- Are you taking any unnecessary medications or can your administration regimen be simplified?
- Are you missing any drug therapy that should be added, to benefit your health?
- How well are the medications working?

- Are all appropriate labs being monitored to keep you safe?

Frequently Asked Questions
Eligibility
Any patient with a Primary Care Provider at CRMC is eligible to meet with a clinical pharmacist. Patients may also be referred by their provider, insurance, or self-referral.
Appointment Details
- • Visits are face-to-face, virtual, or phone call.
- Vists are generally 60 minutes.
- Please bring all medication bottles including prescription, over-the-counter, and supplements.
- Caregivers are encouraged to accompany the patient. • Patients can be seen in Internal or Family Medicine.
- Currently seeing patients only on the Crosby campus.
Insurance Coverage
CRMC will try to bill your insurance for the MTM visits. If the visit is covered, you may be responsible for your copay or deductible. If your insurance does not cover the visit, you will not be charged.
