A team approach

We provide patients (and their families) with a uniquely customized team of doctors, nurses, home health aides, therapists (physical, occupational and speech), social workers and homemakers. The idea is simple: Make sure each patient gets precisely the people and resources they need precisely when they need them. Working together, keeping one another informed, your team is in the best possible position to monitor your situation and provide the best possible support.

Home care

We offer a wide range of essential in-home health programs and services. Whether our home care patients are recovering from an illness or injury, are disabled or chronically ill, we help meet their daily living needs, monitor their well being, and help them to continue to live as independently as possible for as long as possible.

Palliative care

We provide palliative care for patients with advanced or difficult illnesses and limited life expectancies (up to two years). Our palliative care goals are to reduce pain, alleviate distressing symptoms and make the patient comfortable. We can also explain the disease process, and treatment options, and provide patients and families with the information they need to make difficult decisions.

Hospice care

Our hospice program includes medical care, pain management, and emotional and spiritual support for terminally ill patients and their families and loved ones. We work with them to develop and implement a care plan and foster an atmosphere of dignity and peace as patients live out their final days with their loved ones at home.

Our hospice team can include doctors, nurses, social workers, clergy, home health aides, homemakers and volunteers. There are no age restrictions or diagnosis limitations. Bereavement counseling services are available after death.

Lifeline® Peace of mind and 24 hour Emergency Services

For people who live independently, we offer Lifeline 24-Hour Emergency Response. With Lifeline, emergency help is just the push of a button away. The Lifeline subscriber wears a special pendant or bracelet. In the event help is needed, he or she simply pushes a button. A member of Lifeline’s professional staff responds via a small, in-home communicator. The responder assesses the situation, and summons help (a neighbor, family member or ambulance). A Lifeline system can provide a subscriber and his-or-her family the peace of mind they need to continue to enjoy living independently.
