Assess nutritional status and provide diet education to hospital patients as ordered by physician or notification of nutrition risk.

Cuyuna Regional Medical Center Care Center 
Assess nutritional status of all residents upon admission, quarterly and with any significant change of status, assess residents considered high nutritional risk (tube feeding, wounds, dialysis) provide diet specific nutrition education as appropriate prior to discharge home.

Cuyuna Regional Medical Center Outpatients
Provide medical nutrition therapy for patients undergoing weight loss surgery and surgery to alleviate GERD, provide medical nutrition therapy and diabetes self management education for patients with diabetes, teach “I Can Prevent Diabetes” classes, provide medical nutrition therapy for patients with celiac disease, chronic kidney disease, obesity, cancer, and other diseases.

Aitkin

Riverwood Health Care Center
Assess nutritional status and provide diet education to patients as ordered by physician or notification of nutrition risk, provide medical nutrition therapy and diabetes self management education for patients with diabetes, teach “I can prevent diabetes” classes, provide medical nutrition therapy for patients with celiac disease, chronic kidney disease, obesity, cancer, and other diseases.

Aicota Care Center 
Consultation provided monthly.

Community Outreach

Participate in health and wellness fairs; provide lectures to schools, organizations and groups; assist with wellness screenings.

Physician referrals are needed for any outpatient visits.
