Kelly Chase of Nisswa is Cuyuna Regional Medical Center’s new Director of Care Management and Population Health. With more than 10 years of bedside nursing experience and 10 years of leadership experience in nursing and quality improvement, she will provide leadership over care management, community paramedic, social work, utilization review and wellness Registered Nurses.

Chase came to CRMC in May of this year as a Quality Analyst. She has many years of experience leading large and small teams to improve care delivery for patients. Most recently, Chase was at Park Nicollet where she provided leadership in the inpatient and outpatient settings. She also worked in several positions for the University of Minnesota Physicians including clinic manager of a multi-specialty clinic and manager of an outpatient infusion and specialty center focused on the care of solid organ transplant patients.

The Registered Nurse holds a Bachelor of Science degree in nursing and is currently enrolled in a Masters of Health Care Administration program.  She also has completed training in quality improvement, Lean-Six Sigma Green Belt and program management.  She is also certified in Healthcare Quality.
